8)     Loose stones – approx carat weight of the individual stone(s) Mounted stones – approx total carat weight of the stone(s) in each mount
9)     Quantity of Mini reports required
10)  Tick if a short report is required
11)  Tick if a Full report is required
12)  Tick if a sealed report is required
13)  Tick if a verbal grading is required
14)  Tick if laser inscription of the report number is to be applied to the stone(s)
15)  Complete details of the other laser inscription instructions clearly
16)  Tick for required return method.
17)  Total quantity of items submitted for certification
18)  Please state which method of payment you wish to use
19)  Signature of the person submitting the item(s)
